In Cars: The Video Game, Guido serves as Lightning's pit crew during Piston Cup races in story mode, hosting his own mini-game, Pit Stop Challenge. While the first film was being developed, Guido Quaroni gave the character's name as a suggestion to the director John Lasseter and was also asked to do a temporary voice for him, due to originating from Italy. Lasseter agreed to the name, and really enjoyed Quaroni's voice, that he put it into the final film. Quaroni also worked on shadings for the character. They and the rest of the Radiator Springs residents used to attract a number of customers until the Interstate 40 was built to bypass the town, leaving it and Route 66 forgotten. Forty years later, Guido and the rest of the residents are spending their time, when they see a racecar named Lightning McQueen being chased through the town and unknowingly destroying the main road while being attached to the statue of the town's founder Stanley. Guido is cross with McQueen the next day and agrees with Sally that McQueen should fix the road with a road-paving machine named Bessie. By the mention of McQueen being a racecar, Guido is pleased by the fact that he and Luigi are fans of racing, but it soon turns out that McQueen is not a Ferrari, since Luigi only watches Ferrari racing. He also has good skills for performing pit stops, because of his hard practice. His skills are enough for him to make pit stops that only last a few seconds.

In this film, Guido and Luigi tag along with McQueen as his pit crew. He is seen serving drinks at the Wheel Well Motel at the beginning of the film. Later in Tokyo, he doesn't believe Mater's claim that Holley Shiftwell is his girlfriend, until the end of the film where she finally says it herself in a jaw-dropping moment. Uncle Topolino reveals that Guido and Luigi used to always fight; precisely at that moment, they are seen fighting over a girl car until another girl car comes, which solves their problem. He also helps to fight the Lemons later in the film, removing all four tires of two Gremlins in an instant. He tries to take off the bomb attached to Mater, but his drill won't do it and he gets frustrated and leaves. Soon after the race, Guido and Luigi decide to redecorate their shop, when they notice Ferrari racer Michael Schumacher Ferrari arriving in the store with his friends to buy new tires. Luigi becomes so overjoyed to have a real Ferrari in his shop that he faints. Meanwhile, McQueen sets up his racing headquarters in Radiator Springs, attracting customers once again and allowing Luigi to sell his tires once again while spending quality time with his friends. When Guido turns on the air compressor, air will come out of the holes, and the tires will float in the air. There are also some beach balls with the colors of the Italian flag in the ride, and Italian music will play. In Hiccups, Guido tries to get rid of McQueen's hiccups by teaching him to hold his breath for as long as he can.
